I'm afraid I'm starting to get some kind of sex dysfunction but I'm not sure why this would happen

51 y.o. male here. I no my body very well and I take care of it. I workout every other day and I take vitamin and enzyme and amino acid by the handful. I use the Paleo diet, only drink mineral water, eat a lot of eggs everyday, I avoid gluten and lactose as I am sensitive and I only use fruit as a crutch if I have a sugar craving. I have been sober from alcohol and cocaine for 5 years I am 6 foot 2 250 pounds and I have only had 7 sex partners. with this new partner she is 37 and very beautiful but for some reason I keep ejaculating early as if I am a teenager even though my sex drive is very high. my Gout acts up very bad in my left foot to where I can hardly walk when I have sex with her right before her period but other time I always ejaculate early. I am afraid I am starting to get some kind of sex dysfunction but I'm not sure why this would happen because I take care of myself. People think I am only in my early forties. my partner is very nice about it and it helps if I go down on her first because I always make sure shes pleasured first but when she is on top I lose my erection altogether. Does anybody have any ideas?

No nonsense Aidan made an excellent observation: that a diet comprised of lots of meat will contribute to the buildup of uric acid, causing your painful gout symptoms.

"Purines (specific chemical compounds found in some foods) are broken down into uric acid. A diet rich in purines from certain sources can raise uric acid levels in the body, which sometimes leads to gout. Meat and seafood may increase your risk of gout. Dairy products may lower your risk.

"Foods to limit (very high in purines):

Organ meats, such as liver, kidneys, sweetbreads, and brains

Meats, including bacon, beef, pork, and lamb

Game meats

Any other meats in large amounts

Anchovies, sardines, herring, mackerel, and scallops

Gravy

Beer"

And unless you are a body-builder, your BMI indicates you fall into the obese category. Again, not something that should be diagnosed over the internet, so take that with a grain of salt.

You need to go see your doctor and present the entire situation to get proper diagnosis and treatment plans.

IT depends on what’s causing the premature ejaculation and whether this is a new problem or a problem you’ve had all your life. It could be a physical or a psychological issue, or a combination of both. I recommend going to a GP and perhaps finding a sex therapist as well: it’s important that you get expert help to look both at your lifestyle, and techniques for slowing down the time before you ejaculate. One such technique involves stopping when you think you’re getting close to the point of no return, and resuming activity when the feeling subsides, but there are others too and expert advice should be sought.

You said that you have been sober and off cocaine for 5 years – good for you. You might be experiencing sex differently now that you are sober and off drugs, and substances aside, every partner is unique so every sexual experience is unique. You’re not the first person to find a new issue emerging with a new partner and being worried about it, and you won’t be the last. Anxiety itself can make the situation worse, whether it’s premature ejaculation or erectile dysfunction, so it’s important to allow your partner to reassure you and, indeed, to focus on other things that give both of you pleasure as well as intercourse. Try hard not to put yourself under too much pressure as this will only make things worse. I do get the impression that your health overall is very important to you and that’s great, but concern for one’s health can easily take over and become a cause of general anxiety.

One final thing: your choice to follow the Paleo diet is not a good one if you suffer from gout. Meat consumption should actually be cut: it’s one of the chief offenders for encouraging the build-up of uric acid. Also, medications to lower uric acid may help with the gout.
