Dont blame yourself thats the main thing. Ive been with a few guys and half will and half wont. If they dont tell you what works for them how will you know. We're not mind readers.  There are many possible reasons. It could perhaps be your technique but honestly thats probably not it. More likely is that he's not fully able to relax with you giving him oral, and that inability to 'let go' is preventing him orgasming.
The problem is, the longer this goes on, the more stressed he'll get about it because it will become a bigger and bigger block. I suggest you avoid it for a bit and focus on sex where his having an orgasm is not a problem. Then when you come back to it he may be more relaxed about it.
