For the second one, maybe you just got bored of your gf. Try some erotic games, maybe some toys. Or separate for 1-2 weeks and yes, during that time, don't watch porn or masturbate ;)

 If i was you, i would get yourself to the GP asap. This could be a minor thing...but also something major.
It would be in your best interests to get yourself checked out incase it is something serious.
