What do you do if someone is your world...but being together is tearing you both apart?

What do you do if someone is your world. You love them more than anything and you can't imagine ever loving anyone again quite like it. You imagine not being with them, not having a future with them

and it physically hurts. You break up with them and every little thing reminds you of them, and of the future you have lost. You'd do anything for that person, and feel like dying if they should leave you.

But what If at the same time, being with you is hurting them.

Tearing you both apart.

Please help. I love him so much. I can't imagaine life without him. But if I beg him to stay, and we can't sort out our differences, what do I do :(
