It could just be all talk to get the both of you turned on... although I do find it weird that she still talks about it WHILST you're having sex.
My and my fiance do something similar but not all the time. When he's in the mood and I'm not, he'll usually talk about some fantasy of mine or his until I feel up to it. But as soon as we do start to make love he stops. It's purely to get me turned on.
But I think you should talk to her and ask if she actually wants a threesome or if it's just a fantasy. There's no point of you arranging for this to happen if she has no intention of going through with it.
