How do I make his last name mine legally?

My mom and me havent gotten along since my dad left, and that was in the 6th grade. Im 25 now.Shes done so many wrong things to me to where today i have disowned her, i cant forgive her and move on . I have talked to my fiance about my plans to legally change my name so she can never find me. Its just the name that i have chosen is my middle name and his last name. But then when i do that i dont see the need to get married and he is ok with any decision i make. Is it legal to change my last name to his? How much does it cost or does it cost anything?
