Honestly, yes it can depending on how sensitive or insensitive he is to you that first time. A lot depends on how excited you are before he tries as well. You might want to get a small bottle of lube from any local super market or drug store and keep it handy in your purse as that will definitely help your 1st time. They even make a kind that heats up to the touch and will feel a lot less like a medical exam. Remember, don't go there until you really feel you are ready. Good luck.
