Well grammatically the sentence posted would mean : If you already had a 'helper' in Sex!!! If he asked you ,"Do you need a helper in Sex?" that would imply probably his intention for threesome, and that could be mmf, or mff either.

If he used only this sentence then i don't think he mean it. He can make help by anything...
For example pill, Lube, toys, etc lots of things now days people use to have help to have best pleasure.
But if he mension any person or name then its different.
in this case he mean 3some...
But i think u need to talk with him clearly coz u gonna marry him soon...so always try to make sure n clear all subjects...NO CONFUSION allow... coz its harmful for married life...
