What's the best way to go about keeping in touch with a former teacher?

For those who stayed in touch with their former teachers, how do you so (write, phone, meet up or go for coffee with them)?

How often do you contact them? Are you on a first-name basis with them and did you ask for contact details or did they give it to you? Does your way of contacting them depend on whether they are male or female?

I'm sorry that I'm asking so many questions. I can't help it I'm just really curious :)
