 Buy a disposable camera and take her out for lots of adventures - like go to the zoo, or arcade, or any activity that she'll enjoy, like if she's into theater or dance, take her to a concert. Take lots of pictures, fill up the camera and while you take her out to dinner, get the roll developed. Then at the end of the day, you can hand her a whole set of pictures from her amazing birthday!!

ok....you already know that you want to go to dinner...so make a cd with some of her favor songs, found out the highest location where you can overlook the entire city...get a little basket and feel it with some sweet goodies and candles...when that night began go to dinner and let her know that you have more in store for her as the night go on...as you all leave the restaurant blindfold her and tell her that you want to show her a little piece of the world...when you get there tell her to stay put then turn on the music and take her blindfold off....i'm sure she would be amazed by the beauty of the location and the trouble you went through to make sure the night will be unforgettable....hope this help

 You can try an activity of some sort. I don't know whats in your area but for example for my one year with my gf we are going zip lineing. Maybe you could set up some horse back riding, hike, or rent a boat of some sort and go out on the water if there is any near to you. The experience feeling is a lasting one. I wish I could be of more help but I do not know what kind of activities she may like.

 Depending on how long you've been going out, what about a photofame, either with one picture, or even a big clip frame full of tonnes of photo's you could personalise.
You could even give her a limit and say you're taking her shopping for the day, so she can choose what she wants.
Has she briefly mensioned anything in the past about what she's seen? i love it when my boyfriend surprises me and gets me things that i've said i liked, like a new top, something advertised?
A pampering session, where she could get her nails, hair, toes, massage, spray tan or whatever she would enjoy?
I'm out of ideas now, hope they help a little.
