 Why on earth would you even consider sleeping with your ex when you KNOW he has a girlfriend and a baby??? What comes around goes around. I suggest you call him and tell him asap... and get yourself checked out at a clinic, they're easy enough to get rid of. Let it be a lesson to you in the future NOT to sleep with someone else's man!

 You should have considered other people getting hurt before you slept with him. Either way the right thing to do is to tell him. I feel sorry for the girlfriend, he has probably passed them to her.

 By telling face to face. Did he give them to you? If so then his partner also needs to know. Then the best thing you can do is to stay away from this guy. Anyone who cheats on their partner especially when they have a young baby is the scum of the earth, don't get mixed up with him.
Get your health sorted out, then stay away from him and move on.

 When you do things like this people invariably end up getting hurt.
You are going to have to tell him soon, maybe arrange to meet him somewhere and tell him then.
