 Are you sure your penis is erect when you are attempting to have sex, I think you can buy condoms in different sizes, small, medium, large and supersize! I doubt that the condom would have been 'stuck' inside your girlfriend, you could have easily removed it with your finger, however, you probably both panicked and thought it was the right thing to do, but traumatic for your girlfriend, make sure she does a pregnancy test.

Every condom is different - shape, size, lubircant etc... Try a few different ones until you get a fit you like. Look for for "Slim Fit" - they are designed for men with your issue. Also, pay a little more attention while you are having sex; if you feel it start to slip, stop and hike it up. Not a bad idea to reach down with your hand and just verify it is still in place - this may affect your rythym a bit, but that is a small price to pay for avoiding a lost condom or an unwanted pregnancy.
One other option is to look for a cock ring - this is a stretchy ring that goes around the base of the penis. This will help keep the condom on, and also may help you last longer befor eyou finish.

 The ring on a condom is designed to fit around your erect penis, no matter how small (within reason), and it stretches to fit most any size. The length of the rubber doesn't matter because you roll out as much or as little as you need.
It's the girth of your penis that keeps it on. If your penis isn't thick enough to hold it on, you might try another method of contraception. . . or some crazy glue. LOL. Just kidding about the crazy glue.
